We’re so glad you are interested in RAISE Christian Academy! Please fill out the enrollment form below. In order to ensure an opening here at RAISE, you will need to do the following:

• *Schedule a tour

• Complete Enrollment Form

• *Review and Sign the following documents:

  • Financial Agreement (only fill out the ones that apply)

    • Infant / Toddler / Preschool

    • Elementary

    • Before and After School

    • Middle School

  • Parent Handbook (This is part of the enrollment form)

  • Photo Release (This is part of the enrollment form)

• *Submit Applicable Documents

  • Signed Immunization Record

  • School Physical Form

  • Transcripts from Other Schools

• Pay all enrollment fees
